As Riff Raff once said in the iconic Rocky Horror Picture Show, ‘madness takes its toll. In today's Book of Madness 2 slot review, we will be exploring whether this second tome of insanity is as good as the first. Keep on reading to learn more.

Gamomat's Book of Madness 2 is a horror-themed slot game set in an abandoned hospital. Released in September 2025 as a sequel to the first slot of the same name, Book of Madness 2 is another addition to the ‘book of’ genre, which many developers have dabbled with.
Players have to deal with blood-splattered passageways, creepy sounds, and an undead nurse, with all the action taking place on a 5x3 grid with the typical 10 paylines. Accepted wagers on this slot start at 10p and cap out at £100 per spin. Autoplay and bonus buy options are available to those in permitted regions (non-UK).
As fitting for a slot game about insanity, Book of Madness 2 has a very high volatility rating and a hit rate of 30.38% meaning on average a win every 4 spins or so. The default RTP is a respectable 96.11%, and unlike the undead nurses it features, it has a robust and healthy top prize of 11,800x the player's total stake.
There are lots of different online casinos where you can play Book of Madness 2. Here is my pick of the best slot sites:

If you are a gamer, then you will agree that the blood-splattered abandoned hospital walls seen in the Book of Madness 2 slot give serious Dead by Daylight Vibes, while the theme, complete with undead nurses, simply screams classic Silent Hill.
The low-paying royal symbols are also blood splattered, and higher-paying symbols include bandaged undead patients and mutant rats. As is typical with slots of this genre, the wild and scatter symbol is represented by the ‘Book of Madness’, which seems to be oozing some sort of toxic goo. The good thing about the higher paying symbols is that they award payouts starting with just a pair on the reels, as opposed to the usual trio required. The full paytable is as follows:

Ten
0.5x / 2.5x / 10x

Jack
0.5x / 2.5x / 10x

King
0.5x / 4x / 15x

Ace
0.5x / 4x / 15x

Toxic Medicine
0.5x / 3x / 10x / 75x

Mutant Rat
0.5x / 3x / 10x / 75x

Bandaged Patient
0.5x / 4x / 40x / 200x

Undead Nurse
1x / 10x / 100x / 500x
Having been released this year, Book of Madness 2 has been optimised for play on all devices, including mobile ones operating Android, iOS and Windows with no need for any additional downloads.

Despite its creative theme and storytelling, Book of Madness 2 is very light on the bonus features. There is the typical Book symbol, which doubles up as a wild and a scatter, free spins with an expanding symbol, and a simple gamble game.
Like other ‘Book of’ slot games, the book symbol is the most powerful. This wild symbol can replace all others, and as a bonus, the scatter is used to trigger free spins.
Landing 3, 4, or 5 book symbols in a single spin awards the instant win prize of 1x, 20x, and 200x, respectively and will activate the free spins bonus. Players are given 10 free spins, but before these spins are played, one symbol will be chosen by the book at random to become the special expanding symbol. When it appears on the reels to form a winning combination, it will expand three spaces and increase the payout value in the process.
The free spins bonus round can be retriggered, and when this happens, another special symbol will be picked to operate alongside the first, which remains active. This process can repeat until all 9 symbols are special expanding ones.
This feature was a staple of online slot games back in the day, but is seen less often in new releases. Any base game win can be wagered in this optional feature. Taking place on a prize ladder, players have the chance to double or quadruple their winnings or see that number halved or lost entirely.

Book of Madness 2 feels like a game of two halves. On the one side, visually and thematically, the game stands out from the crowd and feels more like a release from more edgy developers like Hacksaw Gaming. On the other side, though, this game is intensely lacking in extra elements. There is no storytelling aspect in the game, and the bonus features are limited to a pretty standard ‘book of’ free spins bonus, and, for some reason, someone at Gamomat blew the dust off the old school gamble feature.
All in all, while at first things look exciting, take away the theming and you have a slot that is a carbon copy of 90% of the ‘book of’ slots out there. The game feels like it is screaming out for some more bonus features like multipliers or modifiers. The only actual gameplay element that sets Book of Madness 2 apart from its many counterparts is its favourable math model, with a 96.11% RTP and 11,800x stake top prize.
With everything considered, I would not nominate the Book of Madness 2 as my new healthcare facility, and it gets a rather limp and lifeless score of 4.5/10.
